Dudley CVS has now secured a total of £124,000 for frontline voluntary sector organisations over two years to support hospital discharge.
Community Bridge is a two-year pilot aimed that will build voluntary sector support around patients being discharged from hospital to help avoid readmission. Dudley CVS will be working with 8 frontline voluntary sector organisations so far to offer the following interventions:
- Settling in at home following discharge
- Counselling
- Welfare advice
- Food parcels
- Spiritual and emotional support
- Peer support groups
- Local walks
- Dog therapy sessions
- LTC self-management support
- Home support packages – sight loss
