Hospital Discharge Link Workers – Social Prescribing
Hours: 37 per week
Salary: £34,434 (aligned to NJC)
Are you passionate about making a difference to people’s lives? Do you have excellent listening skills and are able to work in a fast-paced environment?
Dudley CVS is seeking two individuals to join an exciting new hospital discharge social prescribing service. This exciting role involves working closely with clinical and non-clinical staff to help co-ordinate voluntary and community sector support for patients being discharged from hospital.
You will join a team to provide holistic, one-to-one social prescribing interventions to help patients remain safely at home and avoid unnecessary readmissions. The role also includes supporting community volunteers.
