Hospital Discharge Social Prescriber
37 hours per week
Salary: £38,220 (aligned to NJC PO1)

Dudley CVS is seeking a visionary and proactive individual to help lead the delivery of a new hospital discharge social prescribing service. This exciting role involves working closely with clinical and non-clinical staff to co-ordinate voluntary and community sector support for patients being discharged from hospital.

You will provide holistic, one-to-one social prescribing interventions to help patients remain safely at home and avoid unnecessary readmissions. The role also includes working collaboratively with community volunteers and overseeing a voluntary sector hospital discharge funding programme.
