This event will provide voluntary and community sector organisations the opportunity to explore with partners, the role it can play in supporting the hospital discharge process, specifically where the sector can add value. The event will offer the opportunity to:
- Understand what is meant by hospital discharge and discharge pathways
- Current and predicted challenges
- How we can all work together to improve the situation
- Explore ideas about how in collaboration the ‘bounce back into hospital/revolving door syndrome could be avoided
- Opportunities to surface innovative ideas and solutions across the system
